Custom Sugar Cookie Order Request Form

Thank you for your interest in Sugar Cookie Mama of Sauk County. I can't wait to add some customization to your event! I tend to book at least 3 months out, so be sure to submit your request as soon as possible. I try to respond within 24 hours.

• This form does not guarantee a spot on my calendar. Spots are limited and will be accepted based on availability.

​

• Before filling out the form, please check my calendar for current bookings. If I'm already booked on your preferred dates, I'm likely not able to work with you this time.

​

• PRICING: Standard hand-decorated cookies are $65/dozen which includes up to 4 colors (not including white). Each additional color is $2 per dozen. Minimum order: 2 dozen (24 cookies).​

​

• Hand-filled, 2D printed cookies are $55/dozen.

​

• After accepting your order, I require a non-refundable deposit of 50% to buy supplies and get you on my calendar. Venmo is my preferred form of payment.

• All cookies will be individually packaged and heat sealed to ensure freshness. Pick up is located at my home in Sauk City, WI.

​

• Due to strict copyright laws I'm unable to do characters, sports logos or other trademarked images. Instead I can make something inspired by your theme. If this is something you would like to discuss let me know!

• Cookies are made in a home kitchen that has been exposed to nuts, wheat, dairy and eggs. This home kitchen follows the Wisconsin Cottage Food Laws and is not subject to inspection.
